Summary - Clinical Dietician
Provide nutrition assessment for patients at all levels of nutrition risk, with an emphasis on diabetes education. Oversee development, implementation and monitoring of Medical Nutrition Therapy interventions. Provide patient education regarding therapeutic diets and diabetes self-care regimens at home.
Develop and implement an individualized nutrition plan of care and Medical Nutrition Therapy interventions for patients, while demonstrating awareness for population-specific populations and needs.
Monitor and evaluate patient response to Medical Nutrition Therapy interventions and alter the plan of care accordingly with changes to nutrition regimen/recommendations as appropriate.
Complete all appropriate documentation in the Electronic Health Record, including updates of height and weight, medications, and family history to maintain accuracy of electronic MAR reports.
Participate in departmental meetings and other health care teams to communicate patient care concerns.
Provide dietary counseling and nutrition education, including self-management of diabetes, weight loss, celiac, and other nutritional disorders
Provide good communication between PCP's, patients, and specialty doctors to help facilitate the best patient care.
Bachelor's degree in Human Nutrition or related field required.
Registered Dietitian with the Commission on Dietetic Registration required.
Certified Diabetes Educator
Master's degree in Human Nutrition or related field preferred.
Certified Insulin Pump Trainer with Medtronic, tandem, omnipod, & animas.
Minimum 1 year experience in hospital/outpatient clinical nutrition preferred.
